Qualifications:

  • Graduate from a Licensed Practical Nursing program.
  • Currently licensed as a Practical Nurse in the state of employment.
  • Possesses an active CPR certification.
  • Knowledgeable about specific state laws and regulations governing practice.
  • Satisfactory completion of initial and annual clinical competencies.

A background check, security clearance, and drug screening are required.

Responsibilities
  • Assess the health status of patients.
  • Develop and modify patient care plans.
  • Perform delegated tasks within scope of practice.
  • Provide direct basic care to multiple clients.
  • Ensure safe and effective nursing care.
  • Implement patient education plans.
  • Use problem-solving in decision making.
  • Monitor scheduling and telemedicine appointments.
  • Participate in Quality Improvement and Patient Care Committee meetings.
  • Coordinate follow-up care and offsite visits.
  • Collaborate with medical and nursing leadership.
  • Perform other duties as assigned.
