FeldCare Connects, an app-based referral network, is currently seeking a self-motivated Occupational Therapist to deliver excellent care and work with adults of all ages.

This position is for an Independent Contractor to serve Eljon and the surrounding areas.

The Occupational Therapist in Home Health is responsible for assessing and evaluating patients' care needs related to functionality, activities of daily living, fine motor skills, home assessments, adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment, the Occupational Therapist develops a treatment plan, performs interventions to improve patient wellbeing, and evaluates progress.

Responsibilities include:

  • Providing services as per state laws and in accordance with the plan of care, collaborating with the healthcare team
  • Guiding patients in therapeutic, creative, and self-care activities to improve functioning and safety
  • Monitoring, recording, and reporting patient responses and condition changes
  • Educating patients, families, caregivers, and healthcare team members on therapy participation

Qualifications:

  • Enthusiasm and ability to work with all ages
  • Occupational Therapist license and registration by the state
  • Completion of an accredited Occupational Therapist program
  • Bilingual skills are a plus
