Matrix Medical Network offers a broad range of clinical services and proven expertise that help primary care providers and at-risk health plan members manage health at home. We focus on clinical assessment and care management to improve quality of care, identify chronic conditions, and support members where they are.

During a visit that can last up to one hour, Matrix providers review a member’s health, medical history, medication adherence, social environment, and other risks. The Comprehensive Health Assessment provides insight into overall health and can help close multiple care gaps with a single visit.

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, follow-up, and documentation according to Matrix guidelines and protocols.
• Collaborate with physicians, case managers, social workers, family members, caregivers, and ancillary medical personnel as appropriate.
• Coordinate with the Primary Care Physician on patient education and follow-up.
• Provide services in venues such as Home Visits and Skilled Facility Visits.

• Master’s Degree OR commensurate experience and satisfactory NP licensure
• Current RN and NP licensure in state of practice with prescriptive authority or ability to obtain
• Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
• Current BLS/ACLS or CPR certification
• 1 year NP experience preferred; new grads encouraged to apply
• Strong computer skills; familiarity with medical records software
• Excellent verbal and written communication skills
• Comfortable with frequent change

• Travel required to meet patients where they live
• Valid driver’s license and automobile insurance coverage as required by state

Location: Home, SNF, and other community environments in Brazil, IN

Hours: Days, Evenings, Nights, Weekends; Flexible Scheduling

Benefits: PRN providers are non-benefited except for 401(k) and limited voluntary benefits; mileage reimbursement plan

Earn from $1,800 up to $9,000 per month (based on an average of 20-100 completed visits per month at $90 per completed assessment). Compensation range is based on availability and assessment completion.

Location: Brazil, IN 47834. Primary Location: Brazil, IN 47834. Job: Clinical—Nurse Practitioner (NP). Regular Shift. PRN time. Job Level: Day Job, 8am–7pm, travel, Monday–Friday. No on-call; no evening or weekend hours unless requested.
