Matrix Medical Network is the provider referenced in this role; we deliver in-home comprehensive health assessments and support to members in their homes or care facilities. This position offers flexible scheduling and a chance to impact patient health outcomes through in-home assessments.

• Conduct adult/geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
• Coordinate with Primary Care Physician (PCP) on patient education and follow-up as necessary
• Provide services at the member’s place of residence (homes or Skilled Nursing Facilities)

• Flexibility - Visits scheduled based on your availability; assessments Sundays through Saturdays, 8 am–7 pm
• Competitive pay based on the number of assessments completed; potential earnings vary by state and volume

• Support for additional state licensure and credentialing in neighboring states as needed
• Full-time opportunities available for PRN providers transitioning to FT in home location or region

• Master’s Degree or commensurate experience with NP licensure
• Current NP licensure in state of practice with prescription authority or ability to obtain as needed
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care
• Current BLS, ACLS or CPR certification
• 1 year NP experience preferred; new grads encouraged to apply
• Valid state driver’s license and proof of adequate automobile insurance for state of residence
• Strong computer skills and familiarity with medical record software
• Excellent verbal and written communication skills
