By putting the agency and career growth of Nurse Practitioners at the center of our business, Greater Good Health can expand care to those most in need while giving NPs pathways to becoming holistic healthcare providers.**Community-Based Healthcare**Job descriptionA GGH Annual Health Assessment (AHA) Nurse Practitioner will conduct annual wellness visits and create or update a personalized prevention plan to prevent illness based on current health and risk factors and optimize the management of the patient’s current medical conditions. This will include performing various tests, screenings, providing general and preventative care, and collecting family history information. You will also be responsible for counseling patients with education regarding preventative care and prescribed treatments.Nurse Practitioners will be supporting clients with completing these assessments, partnering with clinics as an embedded part of the care team and/or visiting patients in their homes. NPs will have access to EMRs and/or portals to review charts and document notes. Charts must be completed within 72 hours of patient visit.This position is a full time Nurse Practitioner opportunity (5 days a week, 8am-5pm schedule). This role can include clinic-based, telehealth, or in-home visits. Home visits will be schedule in the Santa Barbara area and we will work with you to keep as much as we can within a 30 mile radius from Santa Barbara, although there will be days where you will travel beyond that radius. This position will be required to flex across regions to provide coverage as needed to accommodate for scheduling changes.**What will make you successful at GGH?**- Highly relationship oriented and ability to passionately serve the aging and elderly population- Thinking critically to optimize accuracy on risk capture, high quality metrics and closure rates- Positive attitude with a solution-oriented approach and a comfort working within ambiguity- Ability to gain trust through communication and integrity- Comfort with technology systems and the commitment to timely EMR documentation- Working knowledge of chronic care management and various medication options- Effectively balances time between patient care and documentation, and exhibits strong work ethic- Ability to travel within the Sacramento area 5 days a week to provide care to patients in their home based on the needs of business/market- Complying with all HIPAA regulations and maintaining security of protected health information**Experience and Requirements**- Current California Nurse Practitioner license- Nurse Practitioner certification from either the ANCC / AANP- 2 + years of clinical experience as an NP in either Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
