Contract: International contractor — $35/hourLanguage: English (mandatory fluency, written & spoken)Reports to: CTO + Engineering TeamAbout the RoleWe are seeking a highly seasoned Senior Backend Engineer with +5 years of experience building and shipping production backends with PostgreSQL. You are an advanced backend engineer with strong database skills, experience integrating external APIs, a strong product focus, and you are comfortable thinking on your feet in a fast-paced and dynamic startup environment. Experience integrating with medical devices and systems is highly desired. Tech StackPostgreSQL (modeling, security, RLS, performance, transactions, triggers)TypeScriptNext.js / Node.jsExternal API integrationsAI/LLM programmatic integrationsAdvanced testing + reliabilityNice boosts: Supabase, DevOps, React, AWS, Python, HL7/FHIR, Temporal/Airflow, medical device integrationsRequiredExpert level proficiency with PostgreSQL Database (modeling, security/authentication/authorization/RLS, performance, transactions, triggers, scaling, data history, etc)TypeScriptNextJS/NodeJSAdvanced experience integrating with external APIs. AI Integration: Experience programmatically interacting with LLMs or other advanced AI modelsSolid CS fundamentalsAdvanced testing experience and maintaining high reliabilityStrongly PreferredExperienced with Supabase (especially Row Level Security and Realtime features)DevOps expertise, implementation experience, and uptime reliabilityAI Training: Capability to help craft our long term AI strategy, along with training and customizing open source AI models based on medical dataSolid proficiency with React (we’re a NextJS shop; since we’re a small team you may be asked to contribute to the front-end)BonusAWSExperience with backend workflow tools such as temporal.io, airflow, etc.Medical device integrations (Immunoassay, Ultrasound, etc)Medical data (i.e. HL7, FHIR & HIPAA compliance)PythonHow We WorkSmall, high-impact teamFast decision-makingHigh autonomy and ownershipClear, async-friendly communicationFocus on real outcomes, not bureaucracy
