As a Intensive Case Manager, you will provide essential services to ensure the well-being of individuals in need. This role involves coordinating various service activities, assisting individuals in developing self-identified goals, and maintaining accurate records.

Key responsibilities include:

  • Developing Individual Service Plans (ISPs) for individuals receiving program services and monitoring progress through regular treatment reviews.
  • Meeting with individuals, families, and other relevant parties to discuss ongoing treatment needs and services.
  • Presenting available treatment options and assisting individuals and families in making informed decisions.
  • Participating in rotational on-call systems to provide emergency coverage and crisis intervention support.

Requirements for success in this position include:

  • Bachelor's degree in Human Services or a related field.
  • One year of experience in case management or a related field.
  • Self-motivation, attention to detail, and ability to multitask.
  • Effective communication and interpersonal skills.
