Evaluate and improve the health and wellness of clients by guiding them toward optimal health within a continuum of chronic disease to well-being.
Key Responsibilities:- Facilitate client behavior change using mindfulness skills and motivational interviewing techniques
- Assess client overall health and determine long-term goals in collaboration with healthcare providers
- Develop and implement personalized strategies to support client health and wellness objectives
- Conduct one-on-one coaching sessions, providing education and instruction as needed
- Bachelor's Degree in Health Science or related field of study
- Minimum 1-3 years of relevant work experience; equivalent experience in health coaching, wellness, or a related field may be substituted for educational qualifications
