Job SummaryWe invite you to provide much-needed care in the comfort of patients' own homes as a Physical Therapist.
Responsibilities:- Help patients maintain, restore, and improve movement, activity, and function through expert physical therapy interventions.
- Conduct regular reexaminations to assess patient progress and modify treatment plans as needed.
- Collaborate with patients, family members, and other healthcare professionals to develop effective discharge plans.
- Perform other duties assigned by management.
- Hold a current license to practice physical therapy.
- Graduate from an accredited physical therapy educational program recognized by CAPTE.
- Possess one year of related experience or approval for less experienced candidates with mentoring and/or participation in a residency program.
- Certified in CPR.
- Enjoy a supportive and inclusive work environment within community-based care centers.
