Home Health Nurse

We are seeking a compassionate and skilled Registered Nurse to join our team in providing high-quality care to patients in the comfort of their own homes.

The ideal candidate will possess strong clinical skills, excellent communication abilities, and a genuine passion for delivering patient-centered care.

About the Role

In this role, you will be responsible for conducting home visits, assessing patient needs, developing personalized care plans, and coordinating with healthcare teams to ensure seamless care delivery.

You will also have the opportunity to work independently, using your clinical judgment to make decisions that positively impact patient outcomes.

To succeed in this role, you should be able to think critically, solve problems creatively, and maintain accurate records.

Responsibilities
  • Conduct thorough assessments of patients' physical, emotional, and social needs
  • Develop and implement individualized care plans tailored to each patient's unique requirements
  • Collaborate with interdisciplinary teams to ensure comprehensive care coordination
  • Maintain accurate and detailed patient records, including progress notes and treatment plans
Requirements
  • Current licensure as a Registered Nurse in the state of employment
  • A minimum of one year of clinical experience, preferably in home health or a related field
  • Demonstrated expertise in current nursing practices, including wound care, medication management, and vital sign monitoring
