Blended Case Manager Job Description

We are seeking a professional to fill the role of Blended Case Manager. This position involves working with individuals who have behavioral, psychological, or developmental challenges, providing support and connecting them with necessary resources.

Duties and Responsibilities:
  • Assist consumers in accessing public and private resources, such as medical, vocational, educational, employment, or other community services as specified in their service plans.
  • Communicate with consumers on a weekly basis to determine progress and revise the service plan as necessary.
  • Support consumers in resolving problems and addressing concerns.
  • Develop and enhance consumer networks of social supports with their families, communities, neighborhoods, schools, and workplaces.
  • Observe and respond to significant changes in consumers' mental and physical health, communicating these changes to the treatment team.
  • Collaborate with county Office of Mental Health personnel and other service providers involved in consumers' treatments to assess needs and provide the most appropriate services.
  • Provide 24-hour on-call crisis services on a rotational basis.
  • Maintain accurate case records and complete necessary documentation, letters, assessments, and summaries according to established policies and procedures.
Requirements:
  • A bachelor's degree in a human services-related field, such as sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, nursing, counseling, or education.
  • Alternatively, an associate's degree with 12 relevant college credits and two years' experience in the mental health/human services field.
    • Excellent communication, writing, and time management skills.
    • The ability to establish and maintain effective working relationships with consumers, families, and staff from diverse backgrounds.
    • A valid Pennsylvania driver's license with a clean driving record.
    • The ability to pass required background clearances and screenings.
