Compassionate Hospice Nurse

This is a dynamic and rewarding opportunity to work in hospice care as a registered nurse. The ideal candidate will have a strong clinical background, excellent communication skills and the ability to work collaboratively with patients and their families.
The nurse will be responsible for performing patient assessments, developing and implementing plans of care, and making referrals to other disciplines as needed. They will also be responsible for maintaining accurate and timely records, teaching patients and caregivers about proper care techniques, and communicating regularly with the clinical team.
Key Responsibilities:- Conduct thorough patient assessments and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Maintain accurate and timely records of patient progress and outcomes
- Teach patients and caregivers about proper care techniques and medications
- Communicate effectively with patients, families and healthcare teams
- Current RN license specific to the state(s) you are assigned to work
- One year of nursing experience, preferably in hospice or palliative care
- Current CPR certification and valid driver's license
- Reliable transportation and liability insurance
