Functional Rehabilitation Specialist

Make a meaningful impact in our home and community program. We are seeking an experienced occupational therapist to provide functional rehabilitation services in various settings, including the client's home, school, work, or community.
About the RoleAs an occupational therapist with our team, you will have the opportunity to work autonomously while being part of a collaborative and dynamic team of therapists. Your primary responsibilities will include:
- Evaluating clients' needs and developing personalized treatment plans.
- Regularly reassessing the effectiveness of treatment plans and attending staffing meetings and family conferences.
- Performing discharge evaluations and making recommendations for home and community safety.
- Communicating with patients, families, and the treatment team to ensure seamless care coordination.
- Collaborating with other disciplines to enhance patient outcomes.
To be successful in this role, you will need:
- A bachelor's degree in Occupational Therapy from an accredited program.
- Current, unrestricted licensure as an Occupational Therapist in your state.
- Current CPR Certification.
- At least one year of experience working as an occupational therapist.
- Demonstrated knowledge of rehabilitation techniques related to complex neurological diagnoses (preferred).
- Excellent communication and interpersonal skills.
- The ability to lift 50 pounds and perform other physical demands of the job.
