We are seeking a skilled and dedicated occupational therapist to join our team in providing high-quality care services to patients in their homes.
The ideal candidate will possess excellent assessment and evaluation skills, as well as the ability to develop effective treatment plans that cater to the unique needs of each patient.
Responsibilities include:
- Conducting comprehensive assessments and evaluations of patients' functional abilities and occupational therapy needs
- Developing and implementing individualized treatment plans that promote patient independence and overall wellbeing
- Collaborating with healthcare teams to ensure seamless care coordination and patient support
Qualifications for this role include:
- Current licensure as an occupational therapist in the state
- Completion of an accredited occupational therapy program
- Strong communication and interpersonal skills
