Home Health Social Worker Job

We are seeking a skilled Home Health Social Worker to join our team. As a Home Health Social Worker, you will work with patients and their families to provide emotional support and connect them with community resources.

Job Description:

The primary responsibility of the Home Health Social Worker is to assess the social, emotional, and environmental needs of patients and develop effective plans to address these needs. This involves conducting comprehensive evaluations, identifying community resources, and providing direct social work interventions. Additionally, the Home Health Social Worker will communicate with healthcare professionals, family members, and other stakeholders to ensure seamless care coordination.

Responsibilities:
  • Conduct thorough assessments of patients' social, emotional, and environmental needs.
  • Develop personalized plans to address identified needs and promote independence.
  • Collaborate with multidisciplinary teams to ensure coordinated care and effective communication.
  • Provide emotional support and counseling to patients and their families.
  • Maintain accurate documentation of patient interactions, progress, and outcomes.
Requirements:
  • Master's degree in Social Work or related field.
  • Licensure/certification as a Social Worker in the state where services will be provided.
  • Minimum 1 year of experience in home health or gerontology settings.
  • Strong communication and interpersonal skills for effective collaboration and patient engagement.
  • Ability to work independently while maintaining strong relationships with team members.
