We are seeking a skilled healthcare professional to join our team as a Registered Nurse. As a key member of the interdisciplinary team, you will provide high-quality patient care and work collaboratively to deliver exceptional outcomes.

Job Duties and Responsibilities

  • Provide direct patient care services that prioritize safety, comfort, and optimal health outcomes.
  • Contribute to patient population management by taking into account cultural sensitivities and local resources.
  • Prioritize patient acuity and clinical flow to ensure seamless care delivery.
  • Recall patients for routine health maintenance as needed.
  • Utilize evidence-based practice guidelines to inform disease management.
  • Participate in direct or indirect care coordination and case management.
  • Deliver patient education aligned with provider care plans.
  • Collaborate with the Care Team to ensure comprehensive services are arranged for patients.

Required Skills and Qualifications

  • Graduation from an accredited Registered Nursing program.
  • Current RN license issued by the State of California Board of Registered Nursing required.
  • Minimum one year of recent experience in ambulatory care preferred.
