Compassionate Hospice Care Specialist

We are seeking a highly skilled Compassionate Hospice Care Specialist to provide high-quality patient care in accordance with established nursing standards and protocols.

Key Responsibilities
  • Patient Care: Provide individualized care plans to meet the unique needs of each patient, ensuring seamless transitions and coordination of care.
  • Communication: Promote patient safety and well-being through effective communication and education, collaborating with healthcare teams to ensure a smooth transition.
  • Requirements: Current RN licensure in BR, experience in hospice or palliative care, strong communication and interpersonal skills, ability to work independently and as part of a team.
