Job Summary
We are seeking a highly skilled and compassionate Registered Nurse to join our Home Health team. As a Registered Nurse, you will have the opportunity to provide direct patient care within the comfort of their own homes, making a meaningful difference in the lives of our patients.
Responsibilities
- Conduct comprehensive assessments and complete OASIS timely to ensure accurate and complete patient care.
- Draft and implement individualized care plans, reconcile patient medications, and ensure orders for care are accurate and complete.
- Prepare accurate, orderly, and timely documentation to support patient care and outcomes.
- Coordinate care with patients and provide regular reports to Case Managers to ensure seamless care transitions.
- Provide health education and instructions to patients and their families as needed, based on assessment and care plan.
Requirements
- Active RN license in the state of practice or eligibility to take NCLEX for new graduates.
