About the Job

We are seeking an exceptional physician to join our team of healthcare professionals. As a provider of value-based primary care services for the senior living industry, we cater to skilled nursing facilities, assisted & independent living communities, Memory Care units, and affordable senior housing sites.

With over 1,000 clinicians serving more than 1,500 senior living community partners across 34 states, we participate in various innovative CMS programs. This role is ideal for a physician who values a flexible work schedule and minimizing uncompensated clinical work.   • Complete health profiles on new patients (minimum rounding once a week)
  • Coordinate with Advanced Practice Professionals to complete/fulfill regulatory and complex patient visits
  • Oversee Advanced Practice Professional performance
  • Ensure exceptional patient care by closing quality gaps when prompted
