Healthcare Ambassador

Astrana Health is seeking a highly motivated Patient Engagement Specialist to enable us to continue scaling in the healthcare industry.

• Conduct outreach calls to inform patients of recommended services for HEDIS measure gap closure.
• Request and gather necessary medical records as needed.
• Effectively organize projects and tasks based on priority to meet expectations and deadlines.
• Collaborate with health plans, PCPs, and healthcare teams to ensure timely communication of member's clinical information.
• Analytical problem-solving with the ability to understand the overall picture.
• Coordinate care for patients as appropriate, including scheduling appointments and creating authorizations.
• Maintain strict confidentiality of business discussions.
• Travel to various offices as designated.
• Ad-hoc projects assigned by the manager/supervisor.
• Comply with department policy and procedures.
• Collect and collate data from IPA network providers for HEDIS measures.

• Reside in Orange County, San Diego, or Inland Empire, CA.
• Minimum Associate's degree, Bachelor's degree, or equivalent years of experience.
• Experience using Microsoft applications (Word, Excel, Outlook).
• Proficiency in electronic scheduling.
• At least one year of HEDIS-related experience.
• Driver's License and reliable transportation required.
• Ability to prioritize and multi-task effectively.
• Excellent organizational skills and attention to detail.
• Effective communication skills.
