We focus on clinical assessment and care management to improve quality of care, identify chronic conditions, and support members where they are.About The PositionDuring a visit that can last up to one hour, Matrix providers review a member's health, medical history, medication adherence, social environment, and other risks.
The Comprehensive Health Assessment provides insight into overall health and can help close multiple care gaps with a single visit.Our opportunities allow you to leverage your expertise and compassion to make a direct impact on health outcomes, with competitive compensation and flexible scheduling.ResponsibilitiesConduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, follow-up, and documentation according to Matrix guidelines and protocols.Collaborate with physicians, case managers, social workers, family members, caregivers, and ancillary medical personnel as appropriate.Coordinate with the Primary Care Physician on patient education and follow-up.Provide services in venues such as Home Visits and Skilled Facility Visits.QualificationsMaster's Degree OR commensurate experience and satisfactory NP licensureCurrent RN and NP licensure in state of practice with prescriptive authority or ability to obtainBoard certification by the AANP, ANCC, or AACCN in a Matrix-approved specialtyCurrent BLS/ACLS or CPR certification1 year NP experience preferred; new grads encouraged to applyStrong computer skills; familiarity with medical records softwareExcellent verbal and written communication skillsComfortable with frequent changeTravel RequirementsTravel required to meet patients where they liveValid driver's license and automobile insurance coverage as required by stateLocation: Home, SNF, and other community environments in Brazil, INHours: Days, Evenings, Nights, Weekends; Flexible SchedulingBenefits: PRN providers are non-benefited except for 401(k) and limited voluntary benefits; mileage reimbursement planCompensationEarn from $1,800 up to $9,000 per month (based on an average of 20-100 completed visits per month at $90 per completed assessment).
