OverviewAccess Services is hiring a Transition Age Youth Case Manager for the Transition to Independence Process (TIP) program serving Lehigh and Northampton Counties, with an office in Bethlehem, PA.What You'll DoAs a Transition Age Youth Case Manager, you will join an elite team dedicated to enriching the lives of young adults aged 16-26 in your community.
The TIP Model is an evidence-based model that actively engages youth through individualized, person-centered, strengths-based planning.
You will work in one-on-one and group settings in diverse community locations.
With a caseload of 15 young adults, you will collaborate with your team to address the unique needs of each person in the program.
Your guidance and support will empower these young adults as they navigate the journey to adulthood, build relationships, access available resources, and develop resilience.We are seeking an individual of high character, a true team player who is passionate about a future career in mental health and social services.
Your strength-focused approach, along with a non-judgmental and motivated mindset, will make you an ideal candidate for this position.Work ScheduleFull Time, 40 hours Monday-Friday.
The schedule offers flexibility, with occasional adjustments based on workload and individual needs.
Some evening and weekend hours are required for special events.RequirementsQualifications of a Transition Age Youth Case Manager:Passionate about helping their community, skilled at engaging diverse populations, comfortable navigating community settings and resourcesExperienced working in the mental health field, and/or with transition age youthComputer proficiencyExcellent verbal and written communication skillsValid license with 2+ years of driving experience with no suspensions, reliable vehicle, and car insurance (required)EducationBachelor's degree, which includes or is supplemented by at least 12 college credits in Human Services, Psychology, Social Work, Sociology, or a related field of studyORA High-School Diploma AND 12-semester credit hours in sociology, social welfare, psychology, gerontology, or other social sciences, plus at least 2 years of work experience providing direct care to people with mental health needsKnowledge, Skills, And AbilitiesExcellent communication skillsAptitude for setting positive, innovative, and goal-oriented objectivesCapacity to independently manage and prioritize tasksCollaborative mindset to work effectively as part of a teamFamiliarity with creative strategies to support young adult mental, behavioral, and emotional healthAbility to engage well with new individuals and develop trusting relationshipsBenefitsWhat we offer:Pay: $ /hour, with opportunity for increases based on performance, education, and growthOpportunity to earn performance-based incentivesOn-call rotation stipend ($175/week)Work laptop and cell phone providedMileage reimbursementOur Full-time Comprehensive Benefits Package IncludesHealth, Vision, Rx, & Dental insurances, HSA/FSA, 401k programTuition Reimbursement20 Days PTO (vacation, sick days), 7 paid holidays, 2 floating holidaysMileage/Toll Reimbursement, paid travel time between worksitesLife Insurance, Short/Long Term Disability, Catastrophic Sick Leave, Paid Parental LeaveEmployee Assistance Program (telehealth/in person)Referral Bonuses up to $750 per hireCollege tuition discounts, Credit Union perks, retail discountsAccess Services is an Equal Opportunity Employer.
