Social Worker/Caseworker (Welfare home)
Location: Woodlands/Buangkok/Bukit Batok
Main Duties and Responsibilities
Daily Duties
- Develop individualise care plans for residents.
- Execute the care plan and monitor progress.
- Coordinate care for residents with relevant stakeholder.
Requirements
- Degree in Social Work, Psychology or Counselling, or a Graduate Diploma in Social Work, Psychology or Counselling.
- Willing to comply with the Standard Operating Procedure when discharging duties.
- Willing to perform shift work in accordance with the Duty Roster.
- At least 1 year of relevant experience.
