Requirements
- Medical degree with a residency or postgraduate qualification in Geriatrics and/or Family and Community Medicine
- Active registration with the Regional Council of Medicine (CRM)
- Previous experience in home-based care or community health is an advantage
- Passion for promoting patient self-care and autonomy through a person-centered approach
- Strong communication and empathy skills
- Ability to work collaboratively as part of a multidisciplinary team
- Proactive in identifying solutions that promote patients’ quality of life and functional ability
Work in alignment with the organization’s values, maintaining a results-oriented approach to care while respecting each patient’s individuality
Record all relevant and essential information related to the care process in the patient’s medical record and keep it up to date
Represent the organization by upholding its principles and providing care based on scientific evidence and best practices
Provide comprehensive patient care with empathy and compassion, building relationships and considering patients’ biopsychosocial needs
Ensure continuity of medical care by implementing the established care plan
Manage the appointment schedule appropriately, fulfilling the working hours and number of consultations established in the contract
