Family Medicine and Geriatrician Physician

• Work in accordance with the organization’s values, focusing on care outcomes and personalized assistance
• Record and keep essential patient information up to date in the medical record
• Represent the organization and provide care based on scientific evidence and best practices
• Deliver comprehensive, empathetic, and personalized care, taking biopsychosocial needs into account
• Ensure continuity of medical care through implementation of the care plan
• Manage the consultation schedule appropriately, meeting the contracted working hours and number of appointments

Requirements

• Medical degree with a residency or postgraduate qualification in Geriatrics and/or Family and Community Medicine
• Active registration with the Regional Council of Medicine (CRM)
• Previous experience in home care or community health is a plus
• Passion for promoting patient self-care and autonomy
• Strong communication and empathy skills
• Ability to work collaboratively in a multidisciplinary team
• Proactive approach to finding solutions that promote patients’ quality of life and functional capacity
• Availability to provide home visits
• Willingness and ability to travel to other cities in the Região dos Lagos region
