Healthcare Access Coordinator

We are seeking a skilled healthcare access coordinator to support licensed providers in managing patient panels.

Key responsibilities include:

  • Establishing relationships with patients and introducing them to the patient-centered care model
  • Ensuring timely treatment and informing patients of their plan of care
  • Coordinating follow-up care and communicating with patients and providers as needed

The ideal candidate will have a strong ability to work with diverse populations, excellent communication skills, and a passion for delivering high-quality patient care.

This role requires a minimum of one year of experience in a related field, preferably in a medical setting.

Education and/or experience requirements include graduation from an accredited Medical Assisting school and possession of current CPR (BLS) certification.

Bilingual English and Spanish language skills are preferred.
