Job Title: RN Care Coordinator

Care coordination is a critical function in the delivery of quality patient care. As a RN Care Coordinator, you will play a vital role in ensuring that patients receive seamless and efficient care across the continuum.

Job Summary:

The RN Care Coordinator will be responsible for coordinating patient care, identifying and addressing health risks, and collaborating with healthcare providers to ensure optimal outcomes.

Responsibilities:

  • Conduct comprehensive intake assessments and develop individualized care plans.
  • Collaborate with healthcare providers to coordinate care and address patient needs.
  • Identify and address health risks, including preventable ED, inpatient, and readmission visits.
  • Provide education and support to patients and families to promote self-management and improve health outcomes.
  • Communicate effectively with healthcare providers, patients, and families to ensure seamless care transitions.
  • Document patient information and maintain accurate records.

Requirements:

To perform this job successfully, an individual must possess the following qualifications:

  • Graduation from an accredited School of Nursing.
  • Associate Degree in Nursing required; BSN preferred.
  • Minimum five (5) years of nursing experience with a minimum of two (2) years in care coordination/case management experience.
  • Knowledgeable about Federally Qualified Health Centers and Value Based Care.
  • Demonstrates strong interpersonal skills.
  • Demonstrates knowledge of Nursing Practice, TJC, and other local, state and federal agencies.
