Psychiatric Case Manager

A mental health professional is needed to provide assessment, diagnosis, treatment, and crisis intervention services for adult and child members who present with a broad range of mental health needs.

Key Responsibilities
  • Coordinate care for patients and assist them in accessing services and resources that will help maintain them in the community in the least restrictive environment.
  • Conduct clinical assessments for interpretation and treatment intervention with patient and families as directed by treatment plan.
  • Provide case management and care coordination services for clients/patients.
  • Intercede on behalf of the individual patients to assist them in achieving and maintaining community stability. Acts as a liaison between patients, referral sources, and funders.
Requirements
  • Master's Degree in Social Work, Counseling, or related field.
  • 1 year experience working in human service field.
  • Ability to remain patient and calm in stressful situations.
  • Must have knowledge of community resources and understanding of how to make appropriate referrals to community and other governmental agencies for services.
  • Understanding of the psychosocial/medical needs of clients in order to formulate an integrated, comprehensive treatment plan.

Physical Demands

Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift.
