A career as a RN Case Manager Hospice offers the opportunity to provide care in a supportive environment.
Key Responsibilities:- Conduct thorough patient assessments and collaborate with the healthcare team to develop an effective plan of care.
- Makes referrals to other hospice disciplines when necessary or documents rationale for not doing so.
- Develops, prepares, and maintains accurate individualized patient care records in a timely manner.
- Documents pain/symptom management changes/outcomes appropriately.
- Teaches patients, families, and caregivers essential patient care techniques.
- Regularly communicates with clinical managers and the care team.
- Provides on-call services and support to patients/families as required.
- One year of experience in nursing.
- Current RN licensure specific to the state(s) assigned to work.
- Valid driver's license, reliable transportation, and liability insurance.
