Responsibilities
  • Conduct thorough patient assessments to identify needs and develop personalized care plans.
  • Collaborate with the care team to ensure effective communication and coordination of care.
  • Provide education to patients and their families on disease management and self-care techniques.
  • Maintain accurate and timely records of patient progress and outcomes.

Requirements
  • Demonstrated experience as an RN in hospice care or a related field.
  • Current licensure as an RN in the state(s) assigned to work.
  • CPR certification and valid driver's license.
