End of Life Care Nurse Job Description

We are seeking a compassionate and skilled Registered Nurse to provide high-quality patient care and support to individuals with life-limiting illnesses.

Responsibilities:

  • Perform initial nursing assessments on new admissions.
  • Develop, coordinate, and revise the total plan of care and maintain continuity of patient care by collaborating with appropriate staff.
  • Regularly communicate patient progress to the clinical manager, physician, and care team.
  • Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness, and according to care center policies.
  • Educate families on patient condition, needs, and care.
  • Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
  • Perform on-call responsibilities and services to patients/families as assigned.

Requirements:

  • Current RN license specific to the state(s) you are assigned to work.
  • One+ year of nursing experience required.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
