Job Summary

We are seeking an experienced Nurse Practitioner to join our hospice team. As a member of our interdisciplinary team, you will provide compassionate and high-quality care to patients in their homes.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans.
  • Communicate with patients, families, and healthcare teams to ensure seamless care transitions.
  • Prescribe medications and treatments as needed.
  • Document patient progress and collaborate with other healthcare professionals to ensure quality care.
Required Skills and Qualifications

To be successful in this role, you must have:

  • A current and unencumbered license as a Nurse Practitioner in the state where services are provided.
  • Minimum of one year experience as a Nurse Practitioner or minimum of one year experience as a hospice Registered Nurse in last three years.
  • The authority to practice as a nurse practitioner in the state where services are provided.
  • Knowledge of Medicare reimbursement and coding for all levels of service.
  • Current CPR certification.
