
Compassionate Hospice Care Provider

We are seeking an experienced Nurse Practitioner to join our hospice team. As a member of our interdisciplinary team, you will provide compassionate and high-quality care to patients in their homes.
Key Responsibilities:
- Assess patient needs and develop individualized care plans.
- Communicate with patients, families, and healthcare teams to ensure seamless care transitions.
- Prescribe medications and treatments as needed.
- Document patient progress and collaborate with other healthcare professionals to ensure quality care.
To be successful in this role, you must have:
- A current and unencumbered license as a Nurse Practitioner in the state where services are provided.
- Minimum of one year experience as a Nurse Practitioner or minimum of one year experience as a hospice Registered Nurse in last three years.
- The authority to practice as a nurse practitioner in the state where services are provided.
- Knowledge of Medicare reimbursement and coding for all levels of service.
- Current CPR certification.
