Job Title

A rewarding career opportunity awaits in the field of hospice care. As a skilled RN Case Manager, you will play a vital role in providing compassionate patient care and supporting families through difficult times.

Responsibilities include performing patient assessments, collaborating with the care team to develop individualized plans of care, and making referrals as needed. You will also teach patients and their families about patient care techniques and regularly communicate with the clinical manager and care team.

  • One year of nursing experience is required, along with a current RN license specific to the state(s) you are assigned to work and current CPR certification.
  • A valid driver's license, reliable transportation, and liability insurance are also necessary.
