This position involves providing essential care to patients and their families in the comfort of their own homes.


$85,000.00-$92,000.00 Annual Salary Range

Responsibilities
  • Conduct thorough patient assessments and collaborate with the care team to develop personalized care plans.
  • Make referrals to other hospice care disciplines as needed or document reasons for not doing so.
  • Prepare and maintain accurate patient care progress records according to care center policies.
  • Document pain/symptom management changes/outcomes as necessary.
  • Teach patients, families, and caregivers effective patient care techniques.
  • Regularly communicate with clinical managers and care teams.
  • Perform on-call responsibilities and provide on-call services to patients/families as assigned.
  • Other duties as assigned.


Qualifications
  • At least one year of nursing experience.
  • A current RN license specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • A valid driver's license, reliable transportation, and liability insurance.
