The RN Case Manager Hospice role is a fulfilling career opportunity in patient care.
Full-Time Days | Patient Care Territory: Schuylkill, Lehigh & Northampton Counties
This position involves providing essential care to patients and their families in the comfort of their own homes.
$85,000.00-$92,000.00 Annual Salary Range

Responsibilities
- Conduct thorough patient assessments and collaborate with the care team to develop personalized care plans.
- Make referrals to other hospice care disciplines as needed or document reasons for not doing so.
- Prepare and maintain accurate patient care progress records according to care center policies.
- Document pain/symptom management changes/outcomes as necessary.
- Teach patients, families, and caregivers effective patient care techniques.
- Regularly communicate with clinical managers and care teams.
- Perform on-call responsibilities and provide on-call services to patients/families as assigned.
- Other duties as assigned.
Qualifications
- At least one year of nursing experience.
- A current RN license specific to the state(s) you are assigned to work.
- Current CPR certification.
- A valid driver's license, reliable transportation, and liability insurance.
