Home Health Nurse Needed

Job Overview

We are seeking a skilled Home Health Nurse to provide high-quality patient care in a fast-paced and dynamic environment.

Responsibilities
  • Coordinates and oversees all aspects of patients' home care, including developing and implementing personalized care plans.
  • Collaborates with patient care teams to ensure seamless patient care.
  • Performs thorough physical assessments, checks vital signs, manages medications, and administers injections, IV therapy, or tube feedings as needed.
  • Supervises LPNs and HHAs, ensuring timely and accurate documentation of patient progress.
  • Communicates regularly with clinical managers, physicians, and care teams to ensure efficient patient care.
Requirements
  • One year of clinical experience as a registered nurse (RN) or approval by VP clinical for less than 1 year of experience.
  • Current RN license specific to the state(s) assigned to work.
  • Minimum six months home health experience.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
  • Able to assess patient status and identify age-specific needs.
  • Familiarity with general computer use, including internet, email, and online services.
