This role involves providing skilled nursing care to patients in the comfort of their own homes. As a registered nurse, you will be responsible for assessing patient needs, developing personalized care plans, and implementing effective interventions to achieve optimal outcomes.

A key aspect of this position is collaboration with interprofessional teams to ensure seamless care coordination. You will also play a vital role in educating families on patient conditions, needs, and care strategies.

Responsibilities include conducting thorough physical assessments, checking vital signs, managing medications, and administering treatments as prescribed. Additionally, you will supervise licensed practical nurses (LPNs) and certified nursing assistants (CNAs).

To succeed in this role, you must possess excellent communication and interpersonal skills, as well as a strong ability to work independently and as part of a team. A minimum of one year of clinical experience as a registered nurse is required, with a current RN license specific to the state(s) you are assigned to work.

The ideal candidate will have a solid understanding of home health care principles and practices, as well as a commitment to delivering high-quality patient-centered care.

Required Qualifications:
  • Current RN license, specific to the state(s) you are assigned to work
  • One year of clinical experience as a registered nurse
  • Minimum six months home health experience
  • Current CPR certification
  • Valid driver's license, reliable transportation, and liability insurance
