This role involves providing high-quality patient care and coordination of services in the comfort of patients' homes. As a Home Health RN, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and collaborating with multidisciplinary teams to ensure seamless care delivery.

Key Responsibilities:
  • Coordinate and oversee all aspects of patients' home care
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
  • Assess patients' physical and emotional needs, and provide education and support to families
  • Perform head-to-toe physical assessments and check vital signs
  • Manage medications, administer injections, and perform other skilled nursing tasks as needed
Requirements:
  • Current RN license in good standing
  • At least 1 year of experience in clinical settings, preferably in home health or hospice care
  • Strong knowledge of medical terminology, anatomy, and physiology
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively as part of a team
