About the RoleThis Hospice Care Specialist position is designed for compassionate, empathetic individuals who are dedicated to providing exceptional patient care. As a Hospice Care Specialist, you will report directly to the Administrator or Patient Care Manager Senior and function under the direction, instruction, and supervision of the Registered Nurse and/or Clinical Supervisor.
Key Responsibilities:- Provide direct patient care, including personal care, grooming, and ambulation support.
- Assist patients with daily activities, such as bathing, dressing, and feeding.
- Maintain accurate records and communicate effectively with healthcare team members.
- Participate in ongoing education and training to enhance skills and knowledge.
- Certified Nursing Assistant (CNA) certification required.
- Previous experience in hospice care or a related field preferred.
- Ability to work collaboratively as part of a multidisciplinary team.
- Strong communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information discreetly.
