Expert Hospice Care Specialist

Our organization is seeking an experienced Hospice Nurse Practitioner to perform critical clinical responsibilities that support the hospice diagnosis and prognosis.
- Gather comprehensive clinical information for the Face-to-Face (F2F) encounter, which will be communicated to the physician responsible for certifying or recertifying patient eligibility.
- Conduct physical assessments within established regulatory time frames and provide findings to the physician for recertification purposes.
Responsibilities Include:
- Performing F2F encounters within scheduled timeframes and notifying the agency upon inability to complete the visit.
- Reviewing medical treatment plans, diagnostic tests, lab results, and medication lists to inform clinical recommendations.
- Making appropriate recommendations to the Interdisciplinary Group (IDG) based on assessment findings.
Our ideal candidate possesses a Master's degree in Nursing, completion of a graduate-level nurse practitioner education program, and current state licenses as a registered nurse and advanced practice nurse. National certification as a Nurse Practitioner and a collaborative agreement are also required.
