Key Responsibilities

Coordinate patient care, continuously assess patient and family needs, and implement the interdisciplinary care plan. Ensure delivery of care is compliant with federal and state regulations.

Requirements
  • Graduate of an approved school of clinical education
  • Licensed in the state where they currently practice
  • At least one year experience as a licensed professional
  • Basic demonstrated technology skills, including operation of a mobile device
Preferred Qualifications
  • Experience as a licensed professional may be deferred with review and approval from regional and executive service line clinical leadership
  • Medicare home health or hospice experience preferred
Additional Requirements
  • Valid state driver license
  • Automobile liability insurance
  • Dependable transportation in good working condition
  • Able to safely drive an automobile in all types of weather conditions
  • CPR certification for healthcare provider
