Exceptional Patient Care Specialist

As a compassionate and skilled Registered Nurse, you will have the opportunity to make a meaningful impact on the lives of patients and their families by providing much-needed care in the comfort of their own homes.
This role requires strong clinical skills, including coordinating patient care, developing and implementing personalized care plans, and educating families on patient conditions and needs. You will also be responsible for supervising LPNs and HHAs, ensuring seamless care delivery and effective communication with the care team.
Key Responsibilities:- Coordinate all aspects of patient home care, collaborating with patient care teams to develop and implement care plans that meet each individual's unique needs.
- Educate families on patient condition, needs, and care, empowering them to provide the best possible support.
- Perform head-to-toe physical assessments, check vital signs, and manage medications, administering injections, IV therapy, or tube feedings as needed.
- Supervise LPNs and HHAs, providing guidance and support to ensure seamless care delivery.
- Regularly communicate patient progress to the clinical manager, physician, and care team, ensuring timely and accurate documentation.
- One year of clinical experience as a registered nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP clinical.
- Current RN license, specific to the state(s) you are assigned to work.
- Minimum six months home health experience.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
