Seeking a dedicated Registered Nurse to join our team in providing high-quality patient care services.


Full-Time Days | Patient Care Territory: Schuylkill, Lehigh & Northampton Counties


This is an excellent opportunity for a compassionate and skilled nurse to make a meaningful impact on the lives of patients and their families.


With a competitive salary range of $85,000.00-$92,000.00 per year, we offer a comprehensive benefits package that includes:

The ideal candidate will have one year of nursing experience, a current RN license specific to the state(s) assigned to work, current CPR certification, and a valid driver's license with reliable transportation and liability insurance.


Responsibilities
  • Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
  • Make referrals to other hospice care disciplines as needed or document rationale for not doing so.
  • Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness, and according to care center policies.
  • Record pain/symptom management changes/outcomes as appropriate.
  • Teach patients, families, and caregivers appropriate patient care techniques.
  • Regularly communicate with clinical manager and care team.
  • Perform on-call responsibilities and on-call services to patients/families as assigned.
