About the RoleThis dynamic position involves serving individuals throughout Northampton County, with an office base in Bethlehem, PA. As a Case Manager, you will be at the forefront of guiding individuals through complex legal and mental health systems with compassion, clarity, and purpose.
You will work directly with justice-involved clients to support their re-entry, stability, and growth, connecting them to vital resources and advocating for their success. This role is about more than casework – it's about restoring dignity, building trust, and helping people reclaim their futures.
Your key responsibilities will include engaging with adults experiencing severe mental illness using person-centered approaches, coordinating care, delivering a warm hand-off to providers, providing de-escalation, and assisting persons being served and team members in increasing awareness of community resources.
To excel in this role, you must have a Bachelor's degree in social work or a related field and experience working with adults with serious mental illness and the criminal justice system. You should also possess excellent communication skills, be able to develop positive, creative, and programmatic goals, direct and prioritize your own work, and be flexible.
The ideal candidate will be knowledgeable about the criminal justice system, serious mental illness, and be able to navigate electronic healthcare records. Proficiency in workplace applications, platforms, and technologies is also essential.
RequirementsThis role requires regular and predictable attendance, and you will be expected to work scheduled overtime or be available for unplanned overtime as necessary. Travel to offsite locations and regional office locations may also be required.
