Job Opportunity

Our blended case management program provides comprehensive support to individuals in need of behavioral, psychological, or developmental assistance.

We seek a highly skilled and empathetic case manager to deliver exceptional service to our consumers.

The ideal candidate will possess excellent communication, writing, and time management skills, as well as the ability to establish and maintain effective working relationships with diverse stakeholders.

Responsibilities
  • Assist consumers in accessing public and private resources specified in their service plans.
  • Communicate with consumers on a weekly basis to determine progress and revise service plans as necessary.
  • Resolve consumer problems and concerns through effective problem-solving and conflict resolution techniques.
  • Develop and enhance consumer networks of social supports in community settings.
  • Monitor and respond to changes in consumer mental and physical health, communicating updates to the treatment team.
  • Collaborate with county Office of Mental Health personnel and other service providers to assess consumer needs and provide appropriate services.
  • Provide 24/7 on-call crisis services on a rotational basis.
  • Maintain accurate and up-to-date case records and complete documentation as required.
Requirements
  • Bachelor's degree in a human services related field (e.g., sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, nursing, counseling, or education) OR
  • Associates Degree with 12 college credits relevant to the position and two years' experience in the mental health/human service field.
    • Valid driver's license with a clean driving record.
    • Ability to pass required background clearances and screenings.
