Lifesharing Coordinator Role

We are seeking a skilled Lifesharing Coordinator to join our team in supporting adults with intellectual disabilities/Autism (ID/A). The role is based at our main office in Bethlehem, PA, and requires regular travel to Lehigh, Northampton, and CMP counties.

As a key member of our Lifesharing program team, you will work closely with provider families and individuals to ensure their needs and preferences are met. Your primary responsibilities will include:

  • Coordinating Individualized Program Plans: Assist individuals in selecting a Lifesharing provider home that suits their unique needs and preferences.
  • Supporting Daily Life Operations: Coordinate all supports outlined in the individual program plan, including monitoring, maintenance, and implementation of compliance regulations.
  • Supervising Support Staff: Oversee support staff to ensure high-quality care and support for individuals.

To be successful in this role, you will require strong organizational and communication skills, as well as the ability to effectively interact with various stakeholders, including county officials, state representatives, and community partners.

The ideal candidate will possess a deep understanding of principles and methods of habilitation, behavior management, safety techniques, and program planning principles. Additionally, they will have experience working directly with individuals with ID/A and a proven track record of advocating for their needs and promoting their overall well-being.
