Key Responsibilities
- Assist consumers in accessing medical, vocational, educational, employment, or other community resources as specified in the service plan.
- Communicate with consumers to assess progress and revise the service plan accordingly.
- Resolve consumer problems and concerns in a timely manner.
- Develop and enhance consumer networks of social supports within family, community, school, and workplace settings.
- Monitor and respond to significant changes in consumer mental and physical health, communicating updates to the treatment team.
- Collaborate with county Office of Mental Health personnel and other service providers to assess needs and provide appropriate services.
- Provide 24/7 on-call crisis services on a rotational basis.
- Maintain accurate case records and complete documentation according to established policies and procedures.

- Strong communication and interpersonal skills for effective collaboration with consumers, families, and healthcare professionals.
- Ability to work independently and as part of a team, prioritizing tasks and managing time efficiently.
- Excellent problem-solving and conflict resolution skills to address complex consumer needs.
- Proficiency in using technology to maintain accurate records and communicate effectively.
