About the Role

This is an opportunity to join a dedicated team of healthcare professionals who are passionate about providing exceptional patient care. As a RN Case Manager, you will have the chance to make a meaningful impact on the lives of patients and their families.

As a member of our team, you will be responsible for performing patient assessments, collaborating with the care team to develop and implement individualized plans of care, and teaching patients and families appropriate patient care techniques.

You will also have the opportunity to work collaboratively with other disciplines to ensure comprehensive care delivery.

The ideal candidate will have one year of nursing experience and current licensure in the state(s) assigned. You will also possess valid CPR certification and a valid driver's license.

About the Position

This is a full-time position working in a patient care territory. The successful candidate will work closely with other healthcare professionals to deliver high-quality care to patients in their homes.

You will have the opportunity to work independently, making decisions based on your professional judgment and expertise. Our team prides itself on its strong work ethic and commitment to delivering exceptional patient outcomes.

Responsibilities include:

  • Performing patient assessments and developing individualized plans of care
  • Collaborating with the care team to ensure comprehensive care delivery
  • Teaching patients and families appropriate patient care techniques
  • Making referrals to other hospice care disciplines as needed

We offer a competitive salary range of $82,000 - $90,000 annually, depending on location and experience. Additionally, we provide a range of benefits, including a full benefits package, paid time off, and up to $1,000 in free healthcare services per year.

Qualifications

To be eligible for this role, you must meet the following qualifications:

  • One year of nursing experience
  • Current licensure in the state(s) assigned
