Hospice Nurse Job Opportunity

This role offers a meaningful impact on the lives of patients and their families by providing much-needed care in their homes.

Responsibilities
  • Perform initial nursing assessments for new admissions to develop comprehensive patient care plans.
  • Accurately document and maintain individualized patient care progress records, ensuring timeliness and attention to detail.
  • Regularly communicate patient progress to the clinical manager, physician, and care team to facilitate seamless collaboration.
  • Educate families on patient condition, needs, and care through clear and empathetic communication.
  • Make referrals to other hospice care disciplines as needed or document rationale for not doing so.
  • Assume on-call responsibilities, providing services to patients/families as assigned, demonstrating flexibility and adaptability.
Requirements
  • Current RN licensure specific to the state(s) assigned.
  • One+ year of nursing experience, with a focus on patient-centered care.
  • Valid CPR certification.
  • Reliable transportation and liability insurance for professional travel.
