IRIS Care Advocate

As an IRIS Consultant, you will play a vital role in empowering individuals in Wisconsin through the IRIS Program. Working remotely, you will be responsible for building meaningful relationships with participants, understanding their long-term care goals, and connecting them to community resources that enhance their lives.

• Client Partnership Development: Meet with participants regularly to identify their goals and develop customized IRIS plans tailored to their unique needs.
• Personalized Support: Explore participants' lives, including their goals, relationships, employment interests, and backup support plans to ensure comprehensive understanding.
• Documentation and Tracking: Document all activities and progress in the IRIS data system within 48 hours to maintain accurate records.
• Community Resource Management: Research and share community resources that align with individual outcomes, promoting informed decision-making.
• Budget Navigation: Process budget amendments and educate participants on reading monthly budget reports to foster financial literacy.

• A bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or a related field is required.
• At least one year of direct experience delivering social services to target groups, including older adults and individuals with disabilities, is necessary.
• Excellent problem-solving, critical thinking, and basic math skills are essential for success in this role.
• The ability to work independently, be self-motivated, and develop professional relationships is crucial.
