Long-Term Care Professional

This role involves working closely with participants to achieve their long-term care goals and connect them with community resources. As an IC, you will build strong relationships with your clients and help them navigate the Wisconsin IRIS program.
The ideal candidate will have a bachelor's degree in social work or a related field and 1+ year of experience delivering social services. You should be self-motivated, able to work independently, and have excellent problem-solving skills.
- Identify participant goals and develop customized plans to achieve them.
- Collaborate with participants to stay within their budget and DHS requirements.
- Research and share community resources that fit individual outcomes.
You will work remotely but have support from our team and coworkers across the organization. We prioritize work/life balance and diversity.
Responsibilities:- Meet with participants a minimum of four times per year, including one annual home visit.
- Provide program orientation, explain rights and responsibilities, verify documents, and complete paperwork.
- Explore participants' lives, including goals, relationships, community connections, employment interests, and backup support plans.
- Assist participants in identifying personal outcomes and ensure ongoing progress within budget and requirements.
- Document all activities in the WISITs data system within 48 business hours.
- Process Budget Amendments or One-Time Expense paperwork as required.
- Educate participants on reading and interpreting monthly budget reports.
- Maintain confidentiality and HIPAA compliance.
- Bachelor's degree in social work or a related field (or commensurate experience).
- 1+ year of direct experience delivering social services.
- Self-motivated, independent worker with excellent problem-solving skills.
